Investigating Assessment for Signal Cases to understand Not-On-Track clients better

While psychotherapy is effective for many, a significant proportion of clients achieve no meaningful improvement. Some even experience symptom deterioration. Routine Outcome Monitoring systems, such as the OQ-System, help identify "Not-On-Track" (NOT) clients at risk of treatment failure. The Assessment for Signal Cases (ASC) further assesses key domains among these clients to guide clinicians: therapeutic alliance (TA), social support (SS), motivation (Mo), and life events (LE). However, prior findings on ASC predictive utility, subscale-level alert patterns, and client clustering have not been systematically replicated. Subscale alert rates largely replicated prior findings, although LE alerts were more frequent in our Dutch sample. Cluster analyses partially supported White et al.’s (2015) typology, with the "internal difficulties" cluster emerging most robustly. Results suggest the ASC has clinical utility but highlight potential cultural and methodological influences on its interpretation.
